To use this sample:

Build the sample. The plugin (VBSample55.dll or CSSample55.dll) and the associated .tbp file are copied to the AutoCAD Map 3D Client bin directory.
Start AutoCAD Map 3D. Two new icons appear on an application toolbar.
After you select one of the items from the menu, that item’s icon is shown on the toolbar. The second button also demonstrates submenus.
