Custom Forms Fields

The following table contains the Custom Forms field descriptions and the location in the module where the fields display.

Field	Description	Location
Author	The contact who created the form.	Main log
Date	The date on which the form was created.	Main log
Date Completed	The date on which the edits were completed.	Edit Custom Form
Favorite Projects	Select the project for which you want to create a form.	Main log
Form	The type of form	Main log
Form Name	Select the form you want to customize.	Main log
Form Title	The title of the form.	Edit Custom Form
No.	The number of the question.	Edit Custom Form
Question	The question as defined in MaintenanceData MaintenanceCustom Form Setup.	Edit Custom Form
