Sometimes a model element can end up in a System View Workset… it’s rare, but it can happen due to a type of file corruption.  In Revit 2012 you can fix this thankfully-unusual occurrence without the help of Revit Support (warning: that you will need to re-enable worksharing and re-make any user-created worksets afterward):

    1. Close the Revit project, and click AppMenu > Open > Project.
    2. In the Open dialog, highlight your project.
    3. In the Worksharing box at the bottom of the dialog, put a check in the Detach from Central checkbox.  Click Open.
    4. In the Detach Model from Central dialog, choose “Detach and discard worksets” (the non-default option).
    5. The file now opens without worksets.   Re-enable worksharing by clicking the Worksets button in the AppFrame at the bottom of the Revit window.

Bonus:  Some Revit customers use this tool prophylacticly and find that occasionally disabling/re-enabling worksharing helps clean their model and improve performance.
